We are looking for a dynamic, motivated Physical Therapist to join our practice full time in Chicago IL 60647. Join our team of passionate therapists and providers as we help restore functionality, mobility, and our patient’s quality of life. We are looking for a Physical Therapist who has experience working with active adults ages 30-60, fantastic ability to connect with patients, and is willing to work alongside a multidisciplinary team to help others.

Duties:
  • Patient evaluation/exam – medical history, review pertinent reports/imaging, and physical exam
  • Treatment plan formulation- Work with advanced medical technology equipment as well as traditional physical therapy to create optimal treatment plans for patients
  • Physical Therapy- therapeutic exercises and modalities  
  • Patient education about the diagnosis, care plan options, lifestyle modifications, and exercises
  • Delegating exercise progressions to rehab techs and oversee rehab floor
  • Collaboration with the other healthcare professionals for comprehensive patient care
  • Documentation/reports- timely and accurate
Qualifications:
  • Graduation from an accredited Physical Therapy program
  • Physical Therapist license in IL
  • 3 + years’ Experience working with 30-60 year old active patients preferred
Schedule:
  • Full time – 4 days / week
Salary (range): 
  • $90k-100k+ per year (DOE)
